COVID-19 Information & Procedures

Office Status

Our office is open. We follow recommended guidelines to keep our clinicians and clients safe, including sanitizing our offices multiple times daily, performing symptom screening on all employees and clients, and temperature checks. 

COVID-19 Hold Harmless Disclosure Form

When you arrive at our office, please note the procedures outlined below for the safety of everyone:

  • Please wear a mask when you enter our office.
  • Please sanitize your hands before you enter our building and in the waiting room.
  • We will be doing temperature checks, and we will ask you some brief questions about any symptoms you might be experiencing.  Individuals with a fever of 100.4°F or greater, coughing,  sneezing, or other Covid-19 symptoms will be asked to reschedule. 
  • If you are a parent, guardian, or transporter that accompanies a client, the same procedure as above will occur. Depending upon the number of people in the office, we may request that you wait in your car during the appointment.
  • Please DO NOT leave the parking lot during the appointment if you are waiting for your child unless approved by your clinician.
  • Please keep proper social distance (at least 6 feet) from all others whenever possible at all times while in the office.

We will recommend rescheduling your appointment and that you seek medical attention if you have:

  • Fever of 100.4°F or higher
  • Coughing
  • Sneezing
  • Severe, constant chest pain or pressure
  • Extreme difficulty breathing
  • Severe, constant lightheadedness
  • Recent loss of sense of taste or smell
  • Serious disorientation or unresponsiveness

Please contact us prior to your appointment if any of the following conditions apply to you:

  • If you have traveled out of the United States in the last 14 days
  • If you have had a fever of 100.4°F or greater within the last 14 days
  • If you live with someone, have had close contact with someone, or have been near someone who has COVID-19.
  • If you live in a long-term care facility (including a nursing home or assisted living facility)
  • If you have visited or worked in a hospital, emergency room, other medical settings, long-term care facility, or jail, or if you were incarcerated within the last 14 days.
  • If any of the conditions above apply to you, you may be asked to reschedule your appointment. 

Telehealth Appointments

We are offering telehealth (telephone and video-conferencing) sessions for therapy and some evaluations. Certain types of psychological testing (i.e., neuropsychological, psychoeducational, attachment & bonding assessments) often require at least some in-person testing.
